These Medical Terms of Service (“Medical Services Terms”) govern your use of the medical services, including the Telemedicine Services described in more detail in Section 12 below (“Medical Services”) provided by Precision Longevity Consultants, PLLC (”Precision Longevity Consultants”, “we”, “us”, or “our”). Please read the Medical Services Terms carefully before using the Medical Services. By using the Medical Services, including the Telemedicine Services, you agree to be bound by these Terms.

Please refer to our Notice of Privacy Practices (available on our website) to learn how Precision Longevity Consultants collects, uses, shares and protects your Protected Health Information (as defined under the Health Insurance Portability and Accountability Act of 1996 or "HIPAA").

Please do NOT use the Medical Services, including the Telemedicine Services, for emergency or urgent medical matters. For all urgent or emergency matters that you believe may immediately affect your health, you must immediately call 911 or go to the nearest emergency room or urgent care facility.

1. Acknowledgment of Experimental Nature and Absence of Established Standards

You acknowledge that longevity medicine is an emerging field. The treatments, therapies, and recommendations provided in this practice may be experimental in nature and may not be based on long-standing medical practices or well-established standards of care. You understand that there is ongoing research in the area of longevity, and the science behind many treatments may be preliminary. As such, the results of these treatments are not guaranteed and may vary depending on individual factors, and there may be risks that are not yet fully understood.

You acknowledge that, unlike many areas of conventional medicine, longevity medicine may not have defined standards of care recognized by medical boards, professional organizations, or healthcare authorities. Accordingly, the therapies and recommendations provided to me are based on the professional judgment of your healthcare provider, and you accept that they may not align with standard medical practices or guidelines.

You acknowledge that some medications, supplements, or treatments may be prescribed for purposes outside of their approved use (off-label). You understand that such uses may not be widely studied or endorsed by regulatory agencies but are recommended by your provider based on the current evidence and professional judgment.

In electing to pursue longevity-related treatments, you understand that you are assuming the risks associated with undergoing therapies for which there may be limited research or regulatory oversight. These risks may include unforeseen side effects, outcomes, or interactions with other medications or treatments. You understand that your participation in these therapies is voluntary, and you may opt out at any time.

You acknowledge that it is your responsibility to fully disclose all relevant medical history, medications, supplements, and treatments to your provider. This is necessary to ensure that the recommendations and interventions are appropriate and safe for your individual health profile.

2. Limitation to Longevity Medicine

The services provided by this practice are limited to the field of longevity medicine and related treatments. This does not include the comprehensive care typically provided by a PCP, such as managing chronic diseases, addressing acute illnesses or injuries, or coordinating care across multiple specialties. While there may be considerable overlap in the services provided, this practice is specialized in longevity medicine and focuses on optimizing long-term health and wellness. You are responsible for maintaining a separate relationship with a PCP for non-longevity focused care.

3. Permission to Treat

You give permission to the Providers to medically care for you. You may withdraw this consent at any time by no longer seeking Medical Services from Precision Longevity Consultants.

You understand and agree that as part of providing Medical Services to you, your Protected Health Information (as defined by HIPAA), including test results, may be released to an online personal health record and via communication with Precision Longevity Consultants’ healthcare team electronically (in accordance with our Notice of HIPAA Privacy Practices).

4. Telemedicine Services and Permission

Precision Longevity Consultants provides Telemedicine (defined below) services to you (the "Telemedicine Services"). You give permission to Precision Longevity Consultants to record and process your personal details and medical data. You may withdraw these permissions at any time by no longer seeking Telemedicine Services from Precision Longevity Consultants**.**

"Telemedicine" is the delivery of healthcare services using technology when the healthcare provider and patient are not in the same physical location, and/or the virtual delivery of healthcare services. The Telemedicine Services may be used for diagnosis, treatment, care, follow-up and/or patient education, and may include, without limitation, the following: electronic transmission of patient medical records, medical images, and/or other patient data or information; synchronous (i.e., "real time") and asynchronous (i.e., non-"real time") interactions via audio, video, text, and/or data or other electronic communications; automated, electronic or digital tools or services for diagnosis, care, treatment and/or communication pertaining to healthcare or medical matters; and output, transmission or exchange of data from medical devices, sound and video files.

You understand that it may be possible that your condition cannot be treated via the Telemedicine Services, or that information transmitted through the Telemedicine Services may not be sufficient or of too poor of image quality, or insufficient information or data to allow for appropriate medical decision making. Accordingly, you may be required to seek additional in-person medical care, alternative healthcare or emergency services. If your health or medical problem or condition persists after use of Telemedicine Services, you will immediately contact your medical services provider and seek appropriate additional in-person medical care or emergency care, as appropriate.
